no problem, I just added the word version to the git repository.
Altough the original is in Apple Pages, the word export opens fine in
Word (as least my instance, which does not necessarily mean too much),
and it opens with slightly distorted formatting, but still readable,
with OOo. HTH.

PS: I tried working with PDF forms once in the past, and it was a lot
of work with little use so I won't try that here again :-)
