I think the real confusing issue is that the syntax description at the very
top doesn't show that <where> is a mandatory argument.  Everything other
than <program> is in square brackets.
In particular, the <where> argument is shown as optional (i.e. it says
[<where>]).

I'm used to looking at the syntax description to see what the mandatory
args, etc... are.
