I haven't heard of an open source option, but Wink is available for free:
http://www.debugmode.com/wink/

Most of the tools will let you do the voice and text annotations, then export 
it all as a swf for
you. If you absolutely have to use open source all along the way, it might cost 
a lot of time
compared to the commercial/closed source options...
