I think that the first approach of creating your own file stream for encryption is an excellent one, but there is a problem (I think). What would happen if the stream's seekg()/seekp() is called. For example, if you wanted to seek to the end of the file, you would need to decrypt the entire file... you couldn't just jump to the end. I guess that's do-able, but would be much longer.
