I can't code the solution for you only point you in the right
direction.  The osgforest example inserts billboards into a scene, use
this as inspriation.  As for do this on paged database, well this is
really something you'll need to explore yourself, I haven't done it
personally.  It should be just a matter of loading a tile, add the
billboards, then saving it.
