That is direction of a traffic sign node, based on direction of the way.
In context of the traffic sign nodes,
direction=forward, or less used traffic_sign:forward=
means the traffic sign orientation is the same as the way
orientation.
direction=backward, or less used traffic_sign:backward=*
means the traffic sign orientation is the opposite to the
way orientation.

Unfortunately, it does not ensure the router interprets it correctly or
at all.
Another issues may be related to the used mapping,
as some sign nodes are not a part of the way they relate to,
and the router may not be aware of them in context of the way segment.
