This can also be due to lack of detail in the OpenStreetMap data for the particular road. And, if that is the case, you can add the data to OpenStreetMap yourself (https://www.openstreetmap.org/, push the "Edit" button to edit) and after OsmAnd+ picks up the changes in its next update, the lane directions will likely improve.
