Hello, Deleting the mvd2 cache directory before running Monteverdi2 is correct.
The program first creates an empty mvd2 cache-directoy before loading the Qt4 SQLite driver plugin. Then, it tries to create the db.sqlite file in the cache-directory. This is the reason why the first error message is about the SQLite driver plugin and the second about the db.sqlite file. I downloaded the Monteverdi2-0.8.dmg package from Sourceforge ( http://sourceforge.net/projects/orfeo-toolbox/files/Monteverdi2/) and installed it on our test platform. I did find the ' /Applications/Monteverdi2-0.8.app/Contents/Resources/lib/qt4/plugins/sqldrivers.dylib' and the application runs fine. Where did you get your Monteverdi2-0.8 package?
