Thanks for the feedback. I am adding a note to the dem parameters of all
applications. I am not sure if we should support setting directly a
file, but anyway this is OSSIM behind the curtains which brings in this
folder thing ... Il will check if we can dot something to support both
folders (useful for SRTM complete database) and individual files.

A quick note to report on the fact that we identified the problem we
had, so that might help others as well: it is caused by the fact that
in the DEM directory there were other subdirectories and OTB was
selecting another tiff file as DEM from the subdirectories! We could
not understand the pattern, this is happening randomly in a couple of
images; it would be wise to include a notice in the OTB
Orthorectification description on the fact that the DEM directory
should be clean and not having other directories in there! It would be
even more appropriate in a future release to select the DEM file
instead of the DEM directory from my point of view.

I am running OTB version 5.0.0 on a Linux Debian 8.2
machine, GDAL version is 1.11.2, however the same issue
occurs when I am running on Windows (OTB via OSgeo4W).
