We have done extensive competancies for OT our hospital- they all check off on modalities, and on any complicated equipment specific to the areas they treat in. All staff have completed competancies in Rehab since they all cover that area. 

Each staff person has a competancy notebook. When we developed the notebook we assigned a competancy to each staff person who developed the materials for it.  Most competancies (espcially modalities and equipment ones) have a competancy checklist, a pre and post quiz, and a handout with the information on the topic.  The staff take the quiz, give it ot the person who developed it, and then get checked off with either the developer or any staff who has passed the competancy.  If an inservice is needed, we videotape it so new employees can watch it, or anyone can watch it for a refresher.
 
Our hospital added non-technical domain competacies last year in response to JCAHO-  they cover problem solving, critical thinking, and age specific criteria.
