Joe - I agree with your take on the therapy world, and I have thought that it would be better ( this does not fit in with my vision for perfect health care) To require something like 1 FTE PT, 1 FTE OT , .5 FTE ST for say 60 beds SNF.  There would be approximately the same ratios for different numbers of beds.  Medicare would pay the cost for these therapists, and the DON would make sure they stay busy - maybe about 80% productivity.  Each therapist is responsible to keep a caseload which allows the therapist to be 80% productive.  If you have great therapy candidates, that might be 4 - 5 patients.  If you are hunting for patients and picking up splinting only patients you might need 10 or so.  Medicare would pay for one OT for so many SNF residents, just like the government pays for one teacher for so many children.  It would not be ideal if you had many good therpy candidates at one time, but I think it would be better with less incentive to game the system than our current system.-- Jody
