Debbie, Medicare requires that a physician certifies the plan of care prior to the initiation of treatment activities. Most SNFs ensure compliance with this by having a clarification order in the chart. It is done because it frequently takes so much time to get the HCFA 700 form signed by the physician. Jimmie
