in Mississippi where I work we have been writing two orders forever.  But we don't call MD a second time.
First order says eval and treat as indicated.
PO Dr Doctor/ J Smith OTR/L
Second order "clarifies" that like 5XwkX4wks for hemiplegia, tx to include ADL, ther ex, ther act, WC mgt to regain PLOF.  Dr. Doctor/J Smith OTR/LSign with MD's name slash my name.  No PO or VO this time.
