Hello David,
In the Sub-acute Rehab/ LTC setting I work at, we use ACLS ( Allen
Cognitive Level Screen test), Routine Task Inventory and Cognitive
Perceptual Test.  SLP's use ABCD to make recommendations. All these are
used to determine best ability to function and to plan best discharge
environment. Rehab "drives" the testing and information gathering
process and presents it to Nursing and Social Services, as well as
patient and families. 
AOTA has Claudia Allen's book. Not sure where to get the ABCD. I'm sure
it can be found through ASHA.

  A standardized OT assessment that addresses the ability to manage
community mobility (among other things) and additionally analyzes the
ability of one to live on their own is the Kohlman Evaluation of Living
Skills.  The KELS can be purchased from AOTA.  I did a search on Google
