Hello: According to Transmittal 36, a doctor's visit is no longer required for outpatient therapy initial certification or for therapy that continues past 60 days, unless the doctor specifically requests it. Is this a correct interpretation? If so, are we still required to report the "date last seen" on the Form 1500??
