Sorry,  I  was  incorrect  in  at  least  one  thing,  the  OPTIMAL is
self-reporting. A therapist does NOT rate the patient's difficulty.

I  must say, the OPTIMAL assessment is vaguely similar to the COPM. On
the  OPTIMAL,  the  patient even identifies their three most important
"activities".

Of  course,  the  COPM  is  not  a Medicare recommended evaluation for
supporting outpatient rehab therapy.
