  Medicare question...
   
  taken from OT practice: "Transmittal 63 requires OTs to include in thier 
documentation of evaluations either the results of a specified performance 
measure tool or an explanation of certain factors. Specifically, the therapist 
may report the results of one of the following measurement tools: the National 
Outcomes measurement system by  the American Speech and Language Association, 
the Patient Inquiry by Focus on Therapeutic Outcomes, Inc,: The activity 
Measure for Post-Acute Care: or the Outpatient Physical Therapy Improvement in 
Movement Assessment Log by Cedaron through the American Physical Therapy 
Association
   
                       Is anybody here on the list using anyone of these forms 
with their pts? I have yet to include any of these forms in my evaluations. I 
wonder if the DASH (disability of arm shoulder hand) and or or COPM would be a 
fit the above requirements?
  I haven't found any place at AOTA or medicare.gov with any useful info yet. 
Any feedback would be great.
