Hello Mike:

I've looked at the OPTIMAL, but haven't used it yet. I do use the
COPM, but not for all my patients. Often, times I use the amount of
assistance required. For example, "patient will transfer to and
from...with min assistance using a standard walker". AOTA posted a
message about addressing CMS about the lack of an OT specific eval.
I've also used depression inventories and dementia screens.

Ron

Subj: [OTlist] transmittal 63: medicare documentation for evaluation

mb> Hello all,

mb> Medicare question...

mb> taken from OT practice: "Transmittal 63 requires OTs to
mb> include in thier documentation of evaluations either the results
mb> of a specified performance measure tool or an explanation of
mb> certain factors. Specifically, the therapist may report the
mb> results of one of the following measurement tools: the National
mb> Outcomes measurement system by the American Speech and Language
mb> Association, the Patient Inquiry by Focus on Therapeutic Outcomes,
mb> Inc,: The activity Measure for Post-Acute Care: or the Outpatient
mb> Physical Therapy Improvement in Movement Assessment Log by Cedaron
mb> through the American Physical Therapy Association

mb> Is anybody here on the list using
mb> anyone of these forms with their pts? I have yet to include any of
mb> these forms in my evaluations. I wonder if the DASH (disability of
mb> arm shoulder hand) and or or COPM would be a fit the above
mb> requirements?
mb> I haven't found any place at AOTA or medicare.gov with any
mb> useful info yet. Any feedback would be great.
