Not  sure  what  you  mean  by  a  coder,  but I frequently bill under a
treatment  diagnosis,  versus  a  medical diagnosis. Often, I don't even
have a medical diagnosis.

There  is  no  such thing as a "treatment ICD code". There are treatment
codes (CPT) and there are diagnosis codes (ICD).
