Rose; I'm sorry, I don't use OTRS 3, I'm running OTRS 5, so I don't know anything at all about how your system should be set up.

Do you have any custom events configured? Does only one agent get multiple notifications, or do all your agents get multiple notifications? How are you changing the owner of the ticket?
