My daughter is 13.5wks old and was doing the same thing. What I did was to detach and reattach her every time she got "lazy" to stop her nipple sucking with a too small a mouth (and hence stop the pain). She eventually got the hint. In addition I also found that she tended to maintain the larger mouth position if I squeezed my nipple when putting it into her mouth so more went in and then supported the breast underneath so what was in her mouth pressed against the roof of her mouth, this made her less inclined to push the excess out and to go back to the small mouth position. It did take about four weeks to get her to take it properly and it wasn't easy for her as I have large breasts and am subject to engorgement when a feed is due and again when I let down, but she eventually got the idea that she got a better longer feed if she just stayed latched ok. She is really good now.
With the sore nipples I found the best thing when mine were a bit tender was just to air them but make sure you have a hand towel handy - I found I tended to drip a lot. But if you have to wear a top (for modesty sake) then the Karicare lotion makes them feel a bit better too.
