Had an interesting discussion along these lines at work today with almost
all midwives from the unit and the CMC (who copped my retort when she
declared babies are DEFICIENT in Vit K!!). They just didn't seem to get my
point about informed consent meaning that women are made aware of the
alternatives to what everyone referred to as the "normal" things that we get
consent for (Synt, Hep B , Vit K), or even that those things are far from
"Normal" and that written info about these things should also include the
"risks" in declining or accepting! I was arguing against the policies that
stated women need to be 'counselled' (ie 'bullied') by the paed registrar
if they decline Vit K.
A comment was rightfully made about labour not being the best time to gain
informed consent , and that should be done antenatally and signed then,
"ABSOLUTELY" I replied to she who loves administering the narcotics... she
who was also rendered speechless when I suggested this should apply to
Pethidine administration, or ARM or scalp electrodes or routine
VE.........and so on.
I love this list, at least I know I am not alone, I do have some kindred
spirits where I work but we are rarely given the opportunity to speak out
loudly. I'm a bit crook today, sore throat etc and as a result was not
sitting quietly, I was a bit cranky and let my passion flow regardless of
that look on their faces.. you know the one, where they are
thinking......"She's a frootloop".(especially when I mentioned lotus
placenta!!)
I don't care what they think, if it benefits the women and babies, and
challenges the other midwives comfort zones then it should be said.
Oh well off to bed now so its not my family that cop my crankiness
tomorrow!!

What is the consent process for Hep B, Are parents aware of the specific
populations of risk?
I must say the Hep B at birth really shocks me. What are the risk factors
for babies who are not in contact with those in high risk groups such as
those already infected or sex workers and intravenous drug users?
It seems like a capture theory to me and I worry about the level of
informed
consent.
