I'd get SP2 on the environment first as its quickly running out of a supported lifecycle.
Is the file being uploaded though the web or SPD? Also check out the file name for illegal characters ( http://www.wssdemo.com/Pages/characters.aspx) Also, what is the extension of the file? Also check the length of the url.
