Is the user trying to upload to the permissioned folder, or are they trying to upload to the library root?

Is the file being uploaded though the web or SPD? Also check out the file name for illegal characters (http://www.wssdemo.com/Pages/characters.aspx) Also, what is the extension of the file? Also check the length of the url.
