The attachement field is very sensitive to changes by sharepoint designer. I'd recommend resetting the form, then start designing it again, but after each modification, save the form and test again to see if the recent change broke it. Figure out which modification breaks the field, then work around it.

It only happens on a particular list, not all lists. Once the list item is created, and the user selects to edit the item, they can now attach a file. Anybody come across this before?
