Wow! Your right.

The difference between the test box and the prod box was that for the original 
web part:

Test:                                                                           
           Prod:
<IsIncluded>false</IsIncluded>                                
<IsIncluded>true</IsIncluded>
<IsVisible>true</IsVisible>                                         
<IsVisible>false</IsVisible>

Changing the newform.aspx in the production box to the values in the Test box 
for the above values allowed the fix to work.

The attachement field is very sensitive to changes by sharepoint designer. I'd 
recommend resetting the form, then start designing it again, but after each 
modification, save the form and test again to see if the recent change broke 
it. Figure out which modification breaks the field, then work around it.

This worked on the test box fine, but on the production box nothing happens 
when the Attach File is clicked, other than the bottom left corner of the 
browser says there's an Error On Page.

Hi

When creating a new list item, clicking the Attach File 
(javascript:UploadAttachment()) doesn't open the page where a user can add an 
attachment to the list item.
It only happens on a particular list, not all lists. Once the list item is 
created, and the user selects to edit the item, they can now attach a file.

Anybody come across this before? I am working on a MOSS 2007 SP2.
