I did this recently with the extra step of switching from MS Agile templates to the Scrum templates.
I used the TFS Integration tool ( http://visualstudiogallery.msdn.microsoft.com/f854dd59-8eeb-4673-8d9a-ae012989bfa2 ) it was buggy. It crashed lots, and needed numerous passes each time creating duplicates. It lost data (or didnt migrate it across). I would have been better to go the alternative route of backing up the SQL database, restoring to the new server, configuring the new users (different domain) and then sticking with the same templates. Going forward we've had to cut and paste missing fields from the old system when we notice they are not there, manually moving over attachments. etc. Worse migration ever, but I had never done one before and was operating blind. Documentation could have been a lot better.
