Failure to grab IP may be because of the controller DHCP proxy feature. 
  I would suggest to put a port on the 3560 in the registration VLAN 
first and see if you get an IP.  If not the problem is either on the 
switch or on PF, if it works, the problem is the controller or we do not 
return the proper VLAN in RADIUS.
