Interesting.. As soon as I click save on the Add Rule screen, the rule is added. I don't need to save again on the source screen. Deletion of a rule requires a save, though. Take a look at /usr/local/pf/logs/packetfence.log when you perform these actions. I had a similar problem that turned out to be the ordering of the authentication sources. If you look at the /usr/local/pf/conf/authentication.conf file, make sure the ldap source appears first.
