I had this same issue. Do you see any errors in radius.log or in
webservices_error_log? In my case it turned out to be needing to increase
num_sql_socks.

Usually pfcmd version doesn't lie (unless it does).
Can you show us the output from these commands?
# dpkg -s packetfence | egrep -i 'package:|status:|version:'
# grep HTTPHEADER /usr/local/pf/raddb/packetfence.pm
There were FreeRADIUS related issues fixed in versions between 4.0.1 and
4.0.5-2.
You might be experiencing one of those.
