I did the updates and it said updated to 4.0.5-2, but i have found things that
were not fixed. So i have fixed code according to the bug fixes. But would like
all the bug fixed.
How should i update on debian?

Usually pfcmd version doesn't lie (unless it does).
Can you show us the output from these commands?
# dpkg -s packetfence | egrep -i 'package:|status:|version:'
# grep HTTPHEADER /usr/local/pf/raddb/packetfence.pm
There were FreeRADIUS related issues fixed in versions between 4.0.1 and
4.0.5-2.
You might be experiencing one of those.
