I do not use radius. I do have AD auth enabled. My setup is also completely 
Inline.

It miss the part with the radius request, do you have something like:
INFO: [f8:2f:a8:cc:8d:a7] handling radius autz request in the log ?
And if yes paste the log arround this line.
