Is there any chance you have httpd_mod_qos enabled?
Check the GUI, under Configuration > Captive Portal.
I don't recommend enabling it unless you are sure you need it and you know what
you are doing.
Also, try to see if you have logs for haproxy.
Check your var/conf/haproxy.conf to see where the logs are sent and make sure
your rsyslog.conf allows syslog over UDP.
It's hard to tell you more without knowing what your configuration is.
The contents of conf/pf.conf , conf/cluster.conf,
var/conf/httpd.conf.d/httpd.portal and var/conf/haproxy.conf would help us
understand the context.
