I thought of that too, but the check in question is a fatal one -- it
exists in the release ROMs as well.

It would be very interesting to see a stack trace.  The line 4350 in Form.c
is just a form validation routine, and so doesn't tell us much except that
someone passed a NULL pointer to the validation routine.  We need to look
up the stack to see who's passing the NULL pointer, and why.
