No and no.  Smells more and more like some memory corruption, in the list
of (open?) databases and/or the memory manager's structures telling which
blocks are locked/inuse/etc.  Try commenting out other bits of code to see
which one might be causing the damage.

I presume you've got Pose doing all the checking possible?
