CategorySelectV10 erroring with object not in form

I have a form that includes a trigger and list. The form handler calls
CategorySelectV10 in response to the ctlSelectEvent for the trigger id.

While running gremlins, I'm now getting an error of object not in form after
about 330,800 gremlins. Anyone have an idea of where to start checking?
Normally, it all works fine (obviously not after 330,800 gremlins!).
