I'm not the original poster but reproduced the problem simply by:
1) Installing CWr6. I have the latest version running (4.0.1 build
0436).
2) Installing the 'sdk35-update1.zip' file which is available from
PalmOS and contains updated PalmOS 3.5 headers.
3) There is an additional update 'PalmPlugin1-6-3b0011.zip' which I
think is available from Metrowerks. I have NOT installed this update
(which simply contains two replacement DLLs that live in the CW
\bin\plugins\Debugger folder). If I DO install them CW will fatally
crash while debugging when you attempt to open the register window.
Please note I simply took a backup of my original Disassembler68K.dll
and PalmPlugin.dll before replacing them with the newer files contained
in the update. The updated dlls crash CW, restoring the older dlls I
backed up makes the problem go away.

The crash happens every time on both Win98 and NT 4 machines.
Happens debugging on either POSE or a real device.

This prevents any possibility of resetting the Program Counter register
