In your case I would remove all related cookies
and try again.

BTW "Independent Developer" is almost one-to-one
Italian equivalent, given the latin origin of both words.

It's there along with the rest of the form. What browser version and
platform are you using? Can you list the fields which *are* there? Try
emptying your browser's cache before going to the page.
