I'm on CW8.1 (I think - IDE 4.2.5.788) and POSE 3.5.  This is
fairly robust for me but I occasionally see the problem you're
talking about.

I haven't been keeping rigorous notes so this is more rumour than
fact but I've noticed it happen after the connection between the
debugger and POSE gets dropped.  This generally means that I'm
debugging a problem with is doing something nasty - sometimes
I'll find that I can't clear a breakpoint and then POSE stops
responding to the debugger - following this the first debug attempt
sometimes fails to load and I have to repeat it.

I'm not sure if it's related to the inane "please wait while POSE
resets" dialog (which waits about ten times longer than is necessary)

To be honest I've started treating it as a sign that one of my segments
is too large and a reorganisation generally fixes the problem, whether
it's actually because I've reset a couple of times and recompiled the
code I'm not sure.

Hmm.  Could I _be_ more vague?
