Which toolbar are you trying to edit?  If you're editing the document 
toolbar, you might have more luck if you remove the path and dirty 
indicator items, then add the button, then re-add those items from the 
"Toolbar Items" group.  With that, I was able to get the toggle command 
button at the right of the toolbar.

I just tried this several times, and I can get it to fail in the way you 
state.  However, as a fallback measure, you can try to remove the IDE's 
preferences entirely.  Just remove or rename the
{My Documents}/Metrowerks/CodeWarrior IDE 4.2 Prefs" file when the IDE 
isn't running.

(Or, you can learn to love ctrl-backtick, which is the keystroke which does 
the toggle source/header operation on Windows.)
