Has anyone out there done this?  What are the charges like from ISP's?  I'm
thinking about distributing an app across the country that uses dial-up
internet access, but we would have to pay for the internet access.

Sure.  Is there specifically something you want to know?  You don't even
have to write an application to play with it, just go into the connection
and network control panels and configure it to use your dial-up card.
Of course, if you want to write a program and set the settings
programmatically, you'll need to look at the NetLib family of functions.  To
set the phone number, etc. you'll want to specifically look at
NetLibIFSettingSet().
