Regarding the license: what you want is a custom licence. You have to write it yourself or pay a lawyer to do it.
You might try to include "no disassembling" and "no reverse engineering" but that might not be enforceable. Last time I checked, reverse engineering and disassembly is still legal in U.S. And that's a good thing. Regarding publishing: there is no "best way" to do it. Most people have ability to create a web page and that's what they do. If you don't you might post it on PalmGear or Handango as freeware.
