Yet another option ... some of the Nolo Press publications have legal forms that might be helpful. See http://www.nolo.com/ I haven't looked at their publication 'Web & Software Development: A Legal Guide' but I've been satisfied with a couple of their other publications. If nothing else, their publications are a good preparation before hiring a lawyer.
