I'm using a Tungsten E, with code warrior. Set the debugger to connect to the palm through the usb port. When you go to debug it it will ask you to enter the short cut .. 2, which is a bit confusing. The short cut is done using the find function (magnifying glass) and the short cut symbol is a lower case written L. On some palms two dots are required (the first beeps the second shows) on others like mine, only one dot is required. So you are typing "l.2" the code warrior software will download to the palm and you are away to the races.

I have found that the usb and palm syncronization seems to get messed up pretty easily so be aware that it may not be your code that is dieing. (Maybe I've just got flakey usb ports).

I don't know the answer to your question but I'm curious what you're using to debug on the palm over usb and what type of palm it is. I am trying to do this with my t5 but can't seem to get it to connect to the debugger.
