Have you run the vcvars32.bat script? This sets up various environment variables, 
including INCLUDE which tells where to look for include files.

I'm using VS 6. On my box the file is in ...\Microsoft Visual Studio\VC98\Bin\vcvars32.

I did nmake clean, then switched to a cygwin box (for the unzip function) and did

$ unzip PAR-0.85-MSWin32-x86-multi-thread-5.8.4.par

Be aware that "nmake clean" wipes out 
         script/parl.exe
         script/parldyn.exe
so the prebuilt binary needs unzipped again after an nmake clean.
Anyway, I then switched back to the Windows command prompt and did
