The problem with Shift+INSERT and JAWS is that JAWS kind of takes over the INSERT key. You have to press INSERT+3, pass key through, to get the Shift+INSERT key combination to work.
The other problem you mention I have no solution for, but I'll add this: In JAWS, the items in the list box are numbered in reverse orver, bottom to top, and in Window-Eyes, they are number one column even numbers, the other odd numbered items.
