It's not free, but I use DVD Audio Extractor from 
http://www.castudio.org/dvdaudioextractor/. It'll extract the audio from 
the DVD and give you MP3 files, as well as other formats. You can have 
all of the chapters combined into one MP3 file or keep them in their own 
separate MP3 files and have a playlist (M3U file) created automatically 
containing pointers to all of the MP3 files. Like I said, it isn't free, 
but it makes the whole process so simple and automated that I found it 
well worth the price.
