If you are using a VGA monitor you are also using a VGA->Mac adapter no? 
If it has dip-switches on it try twiddling then (writing down each 
config for future reference) and see if you can force it to 
1024x768@60Hz. IIRC the Fixed Mode 19" screen was the right setting for 
1024x768 on mine but I don't know about refresh rate, there is one 
setting on mine that forces 1024x768@60Hz but I don't remember what it 
is (I didn't write mine down as they are printed on the reverse of the 
adapter)
