Double check your connections and make sure they are tight.  I ran into a
problem like this once and couldn't figure out the erratic-ness of it.  Then
I finally noticed that the hanging cable from the tower was pulling on the
pci card and not allowing a solid connection.  Are all your screws in and
snug?
