"Apple monitors
The wider connector on your Millennium II card is not a
connector for an Apple monitor. To connect an Apple
monitor, use the adapter supplied with your Millennium II
card. Attach the wide end of the adapter to your Apple
monitor cable and attach the other end to the monitor
connector of your Millennium II card."

The adapter appears to be the usual VGA/Mac adapter.
Strangely, it does not say what the other connector is! :-\

This is the driver I've used. I'm thinking maybe the bios needs to be 
flashed? I did this with my SCSI card and got it operational. It was an 
Adaptec 2940W, now it has a powerdomain bios. Its been abotu 2 weeks
since I 
did that, no data corruption.
